MCH presentation


Here we are (gagjalaxchumi and jiveta ) the 3rd presenters on maternal and child health topic, presented on 21/2/2012.
As you all aware MHC is a very vast topic and very important component of KK. So we plan to present main subtopics only. We have divided our work into two component : maternal health (by jiveta) and child health (by gagjalaxchumi@mimi).

starting off the presentation, with maternal health.................

The objective of maternal health is to promote and maintain the physical, mental, social and spiritual health of every woman in the reproductive age groups, to provide a quality and client  friendly  services to every pregnant woman and to ensure that they receive the optimal health care during pregnancy and undergo a safe delivery process in a clean environment and have a healthy baby .

Maternal health includes antenatal, intranatal and postnatal care which was covered in detail yesterday by Jeevakanthi. It also includes Safe Motherhood this includes antenatal care, family planning, clean and safe delivery and essential obstetric care. Prof Narayan asked me to find more information regarding this safe motherhood and explain it in the next presentation. I did not explain much about family planning as it will be discussed in detail by Wan Teng and Thiba.

We also discussed about risk approach strategy which was implemented to further reduce maternal morbidity and mortality.It  includes training and utilisation of Traditional Birth Attendants, colour coding, Standardisation Of Management Procedure For Obstetric Emergency , Upgrading Of Training & Skilled Personnel, Provision Of Essential Obstetric Care and Health Promotion.

Here we comes to 2nd part of presentation , the child health. Under this topic we covered on subtopics: immediate neonatal care, neonatal jaundice, growth assessment and immunisation, breast feeding. We start off with care of neonate immediately after birth. We list out few basic things done in labour room or OT such as airway, Apgar score, care of cord, care of eyes, care of skin, body temp, breast feeding and measuring the baby birth weight, length and  head circumference.
Prof Kan stopped us and ask ‘ is every child birth take place in labour room or OT’? How about the child born at home ? is apgar score done? After a brief interaction takes place regarding this enquiry  the presentation continues.

Next we move on to 2nd subtopic neonatal jaundice. We presented what is neonatal jaundice , classification and special screening such as congenital hypothyroidism , causes and measurement of serum bilirubin level and treatment. Prof Kan ask us what is the cut off point of serum bilirubin level that have been followed in KK and when the child will be refered to hospital. What parameter is used to measure serum bilirubin level at KK level. What are the clinical method used to assess bilirubin level in KK. During post natal visit to home by JM, what is the thing she does to the baby? How many time postnatal home visit is done? What are the thing they carry with them during the visit?

3rd subtopic of discussion is child growth assessment. Under this heading we talked about growth chart. Briefly we explain component in growth chart such as height and weight measurement, milestone assessment and quick reference on immunisation schedule. We also talked about the inferences that can be elicited from growth chart such as whether the child growth and nutrition status in appropriate, what are the vaccine received by the child so far , whether there is any delay or default  in receiving vaccine, is the child is growing well, slow or overgrowth. Finally but not the list, whether the child fall in sick quite often.
Under this heading Prof Kan pointed out few more questions, such as until what age a child growth is assessed at KK, what action will be taken on vaccine defaulter, How malnutrient child were assessed, handled, screened and managed?. What is additional food basket means, criteria of choosing patient and its contents?

Next subtopic is breast feeding , we explain what is exclusive breast milk means, its content  , feeding method , and when weaning should be started. Prof Kan crossed question us what are the parameter used to assess whether the child is drinking enough milk?  And with that comment the 1st presentation of the day ends and continue with next presentation by our colleague.
