How to plan a family planning?
ans: basically, no matter is a new case or a old case, a booklet will be given which is Kad Perancang Keluarga.

this Booklet is divided into 11 part.
Part 1 : biodata of patient
Part 2: obg and gyn history
Part 3: history of family planning (method, source, year stop)
Part 4 : health condition ( height, weight, BMI, bp, Hb, urine glucose and albumin

 (part 1 till 4 is compulsory for new patient, and the info (the first page) will be send to the LPPKN,KL for the purpose of record and statistic)

Part 5 : Criteria whether the patient is fit to proceed
              -pregnancy, age, number of pregnancy, breastfeeding, postpartum, misscarriage, h/o ectopic
               pregnancy, h/o pelvic surgery, smoking, obesity
              - CVS disorder
              - neurology problem
              - reproductive problem and infection
              - endocrine disorder
              - GIT disorder
              - anemia
              - drug interaction
Part 6 : follow up
            -date, weight, b/p, examination of breast and papsmear (if have), type of contraceptive,
              caunselling, to come again

(part 5 and 6 is compulsory for new patient or patient who change the method of contraception)
